Amendment to Directory Entry

If you would like to apply to have your organisation included in the Broxtowe Help Directory, then please complete and send the form below. If your organisation is considered appropriate for inclusion then you will be sent a more detailed form to be completed and returned.

* The first time you make an amendment to your record you will need to contact us (by phoning 01773 535255 or by writing to Eastwood Volunteer Bureau, Wellington Place, Eastwood, Notts. NG16 3GB) for a security ID number. This must be entered on the form to validate the source of the amendment and prevent others from altering your information.
